Health care professionals often make referrals for counseling for a variety of reasons.  It is surprising how often a patient enters counseling without a good understanding of why they were referred.  I have created a referral form for healthcare professionals that will facilitate communication and give me a good understanding of the signs and symptoms that are of concern.
